After School Programs – General Registration Information

FOLLOW THESE STEPS TO REGISTER

STEP 1:
Fill out and Submit the Online Registration Form.

STEP 2:
Download the Transportation Authorization Form (if we are going to be transporting your child from school to our facility). Complete and sign the form, then deliver it to the U.S. Wushu Center.

STEP 3:
Pay your tuition. You will receive a confirmation e-mail from us with your rates, which you can Pay by Check at time of enrollment.

 

DON’T FORGET THE STUDENT TRANSPORTATION / PICK-UP FORM

If we will be picking your children up and transporting them from their school to our facilities for Wushu After School or After School Care programs, parents will need to download and sign a current Student Transportation / Pick-Up Form.

PAYMENT REQUIREMENTS

Tuition should be paid at time of enrollment. Payment arrangements must be made within 5 days of receiving the confirmation e-mail, or your spot will no longer be reserved.

Please contact U.S. Wushu Center (503-227-3738) and make payment arrangements after you have received your confirmation e-mail. You can pay by credit card or by check. If paying by check, checks or money orders should be made payable to U.S. Wushu Center.

 

REFUND POLICY

Eighty percent (80%) of tuition will be refunded if the student withdraws from the program 7 days before your scheduled start day (by 5:00 pm). After that, NO REFUND will be permitted. We hope you understand that we have a commitment to the teachers, and require a certain number of students in the program for the Wushu program to be successful.
